Must have workers' compensation insurance and liability insurance for at least $300,000 per occurrence and $600,000 aggregate. Federal and Mississippi tax ids required, in addition to a Certificate of Authority from the Mississippi Secretary of State

Fees
$75 for each exam. $200 for a certificate in one classification. Each additional classifications costs $50 each. Certificates good for 1 year
